Cords will be shipped to the name/address of the person identified during checkout if different from the billing name/address. Cords will be shipped to arrive prior to the indicated date needed on the form.

If you did not pay with a credit card at checkout, please include a copy of the Invoice/Order Confirmation with your payment.
